Social Security & Medicare

IRMAA: the Medicare surcharge that can catch retirees by surprise

IRMAA (Income-Related Monthly Adjustment Amount) is an extra charge added to your Medicare Part B and Part D premiums when your income is above certain levels. It’s usually taken right out of your Social Security check, so it lowers the income you actually receive.

2 years

It looks back two years. Your Medicare premiums are based on your tax return from two years earlier. A big-income year at 63 can raise your premiums at 65.

$109,000

Where surcharges begin in 2026: income above $109,000 for single filers, or $218,000 for married couples filing jointly. Thresholds change each year.

1 spouse

The widow’s trap: after a spouse dies, the survivor soon files as single, with half the threshold. Similar income can suddenly trigger IRMAA, adding to the income cliff.

Common triggers

  • Large IRA or 401(k) withdrawals
  • Roth conversions
  • Required minimum distributions
  • Selling a home, business, or investments

Planning ahead can help

  • Time withdrawals and conversions before Medicare’s look-back years
  • Spread one-time income across several years
  • Coordinate Social Security timing with your income plan
  • Request a review after a life-changing event like retirement or the loss of a spouse

2026 thresholds per the Centers for Medicare & Medicaid Services. DB2 Financial Services does not provide tax advice; please coordinate with your tax professional.
